No-Show PolicyAUGUST 1, 2021Thank you for trusting your medical care to Cornerstone Women’s Care. When you schedule an appointment with Cornerstone Women’s Care we set aside enough time to provide you with the highest quality care. Should you need to cancel or reschedule an appointment please contact our office as soon as possible, prior to your scheduled appointment time. This gives us time to schedule other patients who may be waiting for an appointment. Please see our No-Show Policy below. -Effective August 1, 2021 any patient who fails to show or cancel/reschedule an appointment prior to their appointment time will be considered a no-show and charged a $25.00 fee. - If you are scheduled for an in-office procedure and fail to cancel/reschedule your appointment prior to your appointment time a $50.00 no-show fee will be charged. -If a third no-show should occur the patient may be dismissed from Cornerstone Women’s Care. - The fee will be billed to the patient, not the insurance company, and will need to be paid prior to scheduling future appointments. You may contact Cornerstone Women’s Care to cancel or reschedule your appointment at 602-867-2690, Monday – Friday, 8:00 am – 12:30 pm, and 1:30 pm – 5:00 pm. Directions to our New Location
