About Your First VisitMany people are more satisfied with their health care if they share the responsibility with their doctors. Your doctor is an expert on medical care, but you are the expert on yourself. Often there is more than one option for diagnosing or treating a condition. By being a partner with your doctor, you can help choose the option that best fits your values, beliefs, and lifestyle. You also will feel more confident about carrying out the chosen treatment.
During your appointment, you will need to answer some important questions so that you and your doctor can plan your care together. Completing the appropriate forms before the appointment helps you provide correct and current information. Take an active role in your health care decisions, and make the most of your limited appointment time. You will need to bring the following items to your first visit:
Appointments & CancellationsOur office is open Monday thru Friday, 8 AM to 5 PM. You may schedule an appointment thru My Health Record or by calling (602) 867-2690. If you need to cancel or reschedule an appointment you must do so at least 24 hours prior to your scheduled visit.
Our practice generally runs on schedule. However, please be mindful that obstetricians often have emergencies that require immediate attention. You will be promptly notified regarding your appointment and will do our best to minimize any inconvenience. Patient PortalOur online Patient Portal facilitates better communication with our providers by allowing patients convenient and secure access around the clock. Using My Health Record enables you to manage your personal and insurance information; send messages to our clinical staff; manage appointments; review lab results; request a prescription refill.
We encourage you to call our office with any important concerns regarding your health as your well being is our priority. Insurance Health PlansAll of our physicians participate with the same insurance carriers. Our physicians are contracted, in-network providers with the following insurance plans:
Non-Covered ServicesNot all office visits are covered by your insurance policy so it’s important to be familiar with your coverage. You will most likely be responsible for charges such as deductibles,
co-insurance, maximum out of pocket, and copays. If you are unsure about the details of your policy, please contact your insurance company prior to scheduling an appointment. PaymentsAs a service to our patients, we submit all primary insurance claims. We submit secondary insurance claims for patients. Copays are expected to be paid at the time of service. We encourage our patients to contact your insurance carrier to determine what is covered by your plan. Remember that your account is your responsibility even though you have insurance coverage.
Our billing staff is available to assist you will all billing questions. However, to ensure you receive the greatest benefit from your health insurance carrier, we encourage you to become familiar with your benefits including co-payment, co-insurance, and deductible as well as referral and pre-certification requirements.9 Prior AuthorizationPrior authorization is not required for emergency services.
Please note that Prior Authorization is not a guarantee of payment. It is a pre-service determination of medical necessity based on information provided at the time the prior authorization request is made. Cornerstone Women’s Care retains the right to review the medical necessity of services, eligibility for services, and benefit limitations and exclusions after you receive the services. Insurance FormsOur fees are determined by the individual care given and are competitive with area specialists. Not all office visits are covered by your insurance policy so it’s important to be familiar with your coverage.
